3. Get copies of your crash related medical records and send them to me.
CAR CRASH INJURY CASE EVALUATION
I use my more than 30 years of experience to evaluate your car crash injury case. Your case is unique because you are a unique person and every crash involves a different set of circumstances. There is no “book” or “blue print” that can be used to evaluate your case. Evaluating a car crash injury case is part art and part science. The SCIENCE of the case is based on an understanding of the law that applies to issues of liability and damages. The ART of the case is based on my 30+ years of experience and handling hundreds of injury claims to recognize nuances and intangibles that will affect your case.
YOUR CAR CRASH INFORMATION FORM
TERM AND CONDITIONS
By submitting your case for evaluation you are agreeing to the following terms and conditions. You are asking me to review your car crash case and determine if I can accept your case on a contingent fee agreement. You understand that I can only accept a few select cases. By accepting your request to review your case, I am not agreeing to represent you or take any action on your behalf to protect any legal rights or interests that you may have.
North Dakota has a six-year statute of limitations. This means you must commence a lawsuit within six years of your injury or you will lose your right to recover any damages. If your crash happened more than five years ago you must not use this website to request an evaluation. You must contact an attorney as soon as possible to protect your rights to recover damages.
THIS IS STEP 1:
Complete and submit this Case Evaluation form. —->
After you have provided all of the required information I will complete my evaluation of your case as quickly as possible. I will contact you as soon as I have completed my evaluation.
Until I accept your claim I will not take any action to protect any legal rights or interests you may have. You cannot expect me to take any action concerning your case until I send you a letter confirming that I have accepted your case and you have returned the signed retainer agreement to my office. If I cannot accept your case I will send you a letter to the address that you have provided. If I do not accept your case it does not mean that you do not have a valid claim. I strongly recommend that if I cannot accept your case that you contact another attorney to review your case as soon as possible.
STEP 2: SEND ME A COPY OF YOUR CRASH REPORT
I cannot complete my evaluation of your claim until you provide me with a full copy of your crash report. Please send me a full copy of your crash report as soon as possible.
The Officer that investigated your crash prepared your crash report. You may have talked to the Officer at the scene of the crash or at the hospital.
You may be able to obtain a copy directly from the City Police Department or the County Sheriff’s Department. Go to the Police Department or the Sheriff’s Office and request a full copy of your crash report that includes the officer’s notes and description. There may be a small fee charged to obtain a full copy of the report.
If your crash was investigated by the Highway Patrol you will need to request a copy from the North Dakota Department of Transportation or the Minnesota Department of Public Safety.
When completing a request it will ask for the “reason” that you are requesting the report. Put “Claim Investigation” as the reason for requesting the report. After you receive a copy of your crash report mail it to me at:
1336 25th Avenue South #202
Fargo, ND 58103
Or scan a copy of the report and email it to me at: email@example.com.
